1. You have traveled outside the United States or recently or had close contact with someone who recently traveled outside the state of Ohio and is ill AND/OR you have a fever, cough, sore throat, shortness of breath or body aches.
2. You have traveled outside of the state or country and returned to Ohio within the last 14 days.
